After returning home, you may need to understand what happened during treatment, give a local clinician accurate information, or ask a Turkish lawyer to review a concern. A complete file helps those people work from the same chronology. It can also help distinguish what was planned, what was performed, what was recorded afterwards, and what follow-up was offered.
A record request is a practical first step, not a conclusion that malpractice occurred. A complication or an unsatisfactory result does not by itself establish legal fault. The medical records need to be considered alongside the treatment plan, consent discussion, clinical context and later care.
Article 16 of the Patient Rights Regulation provides that the patient may inspect records about their health and obtain a copy, either personally or through a duly authorised representative or legal representative. The regulation also covers public and private health institutions. The facility’s own identity-check and delivery procedure may still need to be followed.
What to ask the clinic or hospital for
Request the records connected with the relevant assessment, procedure and follow-up. Use a list so that a short summary or discharge sheet is not mistaken for the full file.
- Consultation notes, medical history, examination findings and pre-operative assessment.
- All consent forms, patient-information documents and versions supplied in your language.
- The operation or procedure report, including the date, facility and names or roles recorded for the treating team.
- Anaesthesia records, medication charts, nursing observations and recovery-room notes, where applicable.
- Laboratory results, imaging, photographs and other test results that form part of your file.
- Discharge instructions, prescriptions, aftercare notes and messages documenting follow-up or referrals.
- Invoices, itemised payment records and the name of the legal entity that issued them.
Ask for a complete copy in a readable format and, if the facility offers it, an index of the documents supplied. If a category does not exist or is held by another provider, ask the facility to identify that clearly rather than assuming it was omitted.

If you need a Turkish version, ask a fluent translator or counsel to translate the request accurately. Keep the original and sent version together.
How to send and track the request
- Verify the provider name and official contact route on your invoice, discharge documents or the provider’s official website.
- Send a dated written request and save the exact message, attachments, delivery confirmation and any reply.
- Ask whether the records can be provided electronically and how the facility will verify your identity.
- If another person will collect the file, ask what signed authority or representation documents the provider requires.
- When a response arrives, save the original files without editing them. Make a separate working copy for translation or annotation.
Keep a simple log with the date, recipient, channel, response and any documents received. If you receive only a partial file, reply with a short list of the missing categories and ask whether they are held elsewhere. What to do with the records once they arrive
Preserve the files as received, including email headers, download links and any accompanying index. Store a backup securely. Do not overwrite the originals while adding notes; create a separate chronology that points back to the source file and page number.
If the documents are in Turkish, first identify which items need translation for a clinical second opinion or a legal review. A court may require a Turkish translation of foreign-language documents submitted as evidence, and may ask for an official translation.
